Either someone didn't do the proper headspace checks or they failed to perform proper PMCS.
Good way to damage a barrel though.

Preventative Maintenance, Checks and Services
PMCS is what you do when you clean, lube, maintaintain and operate any piece of miltary equipment. The proper PMCS is found in the TM (Technical Manual) or FM (Field Manual) for every pice of military hardware. Here's the FM for the M-2.
