At least with the 9mm magazines, you can install the spring so its not seated correctly and the plastic 'pusher' will get cocked a little sideways at the top of the travel preventing the slide release from being pushed up to catch the slide.

When it fails to lock open, pull the mag out and look at it...is the little lip that is supposed to push the slide stop up sitting high enough in the magazine to push on it?

Everything regarding the magazines is on the up and up. the springs and followers are installed correctly.

as i have never demonstrated nor have been demonstrated a limp wrist discharge i am oblivious to it if that is indeed what's happening. but i like to think i have a firm grip on the pistol.

You can have a tight grip on your XD and still be limpwristing. If you let your wrists and/or elbows act as shock absorbers during firing, it can cause cycling problems.

Also, if your thumb is riding the slide (not touching any of the controls), you can slow the slide enough that it won't stay back after the last round.

Have someone else shoot it.

If it still happens, time for a trip to SA.

But don't you feel great that it's an easy fix and not something wrong with the pistol?
