I've heard (read) some folks say that a slightly loose grip will allow a malfunction on the XD-45.

I am recovering from many years of neglecting my wrist (old injury never really healing), and mine cycles OK, but I've read of folks having issues.

Maybe an over powered slide spring and a loose grip on the pistol?

Keep shooting it takes 400-500 rds to break it in or to make your wraist stronger. I'm right handed and my left hand has to be strong wraist lock to keep it from stove piping. Keep shooting.

When I first bought my compact, the empty 10rd magazine wasn't fitting very well at all... and it turned out the magazine follower had just gotten knocked a little out of place... so I just had to make sure it was seated in properly.

I'd say it's probably your mag.

...and after 200 rounds no malfunctions other than the slide failing to lock open when empty once.

Been only using the Blazer Brass 45 acp in my Service XD45, no problems in 500 rounds.

For the sake of argument, have to ask...How long has it been since you've fired a .45 and any chance the malfunctions occurred due to technique / limp wrist???

Since I purchased this .45 it has been maybe 8 yrs. I spent some time doing double taps at the range. My follow up accuracy could use some improvement which means I'm not ruling that out. But I would not say my wrist is "excessively" loose. But I better start going twice a week just to be sure. Never can be too sure about these things. At least that's what I'll tell the misses when she asks why I'm going through 400 rounds a week.
