A couple folks got it right. Bullets begin dropping the moment they leave the barrel. If you draw a line across the sights and continue it out to infinity and then draw a line from the barrel out to infinity (and they will NOT be parallel), the point at which they intersect will roughly be where the pistol is sighted. What this means practically is that the bullet "rises" to its sight-in distance (although in reality the barrel is simply aimed upward in relation to the sights), continues above the sight-in point, and then at some point drops back to the sights before dropping below the sight setting. So the sights will actually be "on" at two distances.

As to long range handgunning...read Elmer Keith's works to see what is possible. I personally enjoy the meals I've won betting I can hit man-sized targets at 200 yards with "combat pistols".
