Charlie,
IF you are shooting reloads and IF these reloads are loaded with a light charge, the problem could be the recoil spring. The XD has a heavy recoil spring and shooting lighter loads in it can result in the slide "short-cycling". If the slide does not cycle fully, it is possible for it to eject the empty but not fully/properly catch the next round. The slide would then push the round against the front of the mag and the only direction that the round can go is down into the mag. The XD's recoil spring is heavy enough for it to handle 45 super loads. If your shooting light loads, the recoil spring should be changed to a lighter one. An easy way to see if the recoil spring is the problem, assuming you using hand-loads, is to load up a hotter batch. Bring up the muzzle velocity to 900 fps and see if the problem still exists.

Try this at next range visit. Load a mag of jacketed ball. Fire one round and hand eject the next. Fire one round and hand eject the next. Do this until the mag is empty and you have half the rounds sitting on the bench or on the ground. These rounds have been through the reloading cycle. Now look at them closely. Do they have the mark you are talking about but not as deep ? If so then it is a function of the loading cycle. The lead bullets you are using are too soft to overcome the impact and are sticking. Try a tempered cast lead bullet and see if the problem clears up. My XDm leaves a very nice half crescent in a chambered Reiner round. If it were not plated I believe it would stick if under 20 BHN lead.

I know it's two years old. I'll rephrase the question: two years on, has Springfield Armory done anything to address this problem in the XD .45, other than advise against semi-wadcutters and keep replacing magazines?
