I sent an email to the owner of the range yesterday asking about the incident and if he could shed any further light on the matter as I would honestly like to know and so wouldn't my wife who has a 686 on order.

Don't worry about the 686 you ordered for your wife. It'll be fine.

Squib bullet stuck in the barrel? If so, the employees apparently don't have a set of rules to follow when the rentals are turned in.

the 686 is a powerhouse, very well made.
Is likely neglect of the range, probably never clean the rentals

Yeah I agree probably neglect on range's part, improper cleaning and inspection. My guess is the wheel wasn't lined up with the barrel from abuse due to it being a rental (which should be checked whenever a rental is returned or rented out). On the bright side it was probably a good thing you started with 38 special ammo. Glad everyone was OK!
