I've seen two AKs explode though luckily I wasn't the one firing them. One was in the states and one was in Iraq. I also had a Sig Mosquito basically fall apart me.

My yes vote is from a machine gun shoot at Fort AP Hill. My unit didn't have a range on base, so we had to travel to AP Hill. We were shooting 240G machine guns at a bunch of hulks at unknown distances, with people calling out the "threat". I was assistant gunner in the rotation when the gun spit out the feed pawls that pull the belt of ammunition into the gun.
I cracked it up to the new armorer probably didn't put it together right, especially since the shotgun I had stored in the armory somehow lost an internal spring.

In 1989 I went hunting with 4 friends in Michigan. I had a Savage 30-06 but wasn't comfortable with it so a friend lent me his Remington 700 30-06. After the first day of hunting we all gathered back at his house and during unloading the rifle, it fired inadvertently. I thought I had my finger no where near the trigger but everyone came to a conclusion that I must have had my finger on the trigger while cycling the bolt.
About 20 years later I watched a TV show on CNBC that stated this EXACT thing has happened hundreds of times with this series of rifles.
I guess I consider this a major problem with this firearm although it wasn't mine, it did happen to me.

I was not shooting, I was Coaching during USMC annual pistol range qual and a Major's lockingblock sheared off a lug during firing, when the slide extracted the round. Pieces of it were everywhere, but the slide returned to battery...with another round...and the slide wouldnt come to the rear to go,condition 4. Ended up taking the slide off with the round still in, removed recoil spring and operating rod, followed by careful removal of locking block pieces, barrel and round. It was a REALLY old and out of repair M9 with a 1st generation locking block. 4th gen will last longer than the frame.

Unless the firearm comes stock with a buffer of some sort, I have never heard of them doing any good.
They tend to break apart and get lodged into every place imaginable. Which can cause serious issues. I honestly don't see how they can legally sell them.

You got lucky! Count your lucky stars. And yes, this plagued early 700s. The new ones don't have this issue, though.

My XDm .40 4 1/2", my favorite gun.
In Feb. it broke the the hook off the (PRP) trigger spring. I then had a hammer. 
A couple of weeks ago during a MAG 40 qualification course the back tip broke off the disassembly bar. The gun would not cock. I then had a hammer. 
And a revelation! I should carry a backup.

Had a CX4 blow out a case in the chamber breaking parts in the trigger that required a trip back to the factory.
