Imagine if he is doing a Mexican carry!

User error but happens a bit too often to ignore. I heard similar also getting caught/snagged on a shirt while holstering (on a kydex holster).

There is no probably. The holster was worn and faulty, and should have been replaced long before it got this point. BTW--folks have had ND's with 1911's and XD's as well...they aren't idiot-proof.

Folks, no mechanical safety is going to save you if you don't pay freakin' attention to what you're doing. Blaming the pistol becaase the owner was too complacent to keep their gear in shape, or replace it when it got worn is ludicrous.

Who is blaming the gun? All I see are comments favoring additional measures of safety, such as checking your gear and grip safeties, etc.

Exactly...You are responsible for your gear. That holster has been like that a long time.. It should have been discarded ages ago.
