I use a horizontal shoulder rig w/ my Loaded 1911. Locked and cocked is the preferred way by the majority for combative readiness.

Cocked and locked in a Galco Summer Comfort IWB. I have a 5" Kimber Custom II and it is comfortable and very concealed. I am a medium build and the gun barely prints in a T shirt.

Condition 1: Cocked and locked
If you carry with a round in the chamber and the hammer resting on the loaded chamber (condition 2) you seriously risk having a rear strike the hammer drop on the firing pin. Do not do it. Very unsafe. As others have mentioned, carrying without a round in the chamber requires you to rack the slide first. This could become a nearly impossible feet when under attack. Literally, ever since 1911, that's 97 years, the design has been thoroughly tested. You are perfectly safe to carry in condition 1. The gun will not fire unless something pulls on the trigger while the safety is disengaged and the grip safety is depressed. However, when holstering, I still tend to hold the hammer with my thumb, but consciously make sure I do not depress the grip safety (similar to how I holster an XD).
