Mystery problem

Ok this is what happened to me to the last IDPA match a couple of days ago. I was doing pretty good up until the last stage, where while performing a slide lock reload with a Springfield 1911 loaded, the weirdest thing happened. I hit the slide release, bang, then click. I tap, rack,nothing happened. I pull the slide back and I see the top round on the mag sticking out half hay preventing the mag from dropping free. So i rip that mag, insert another and continue only now the slide doesn't lock back because the slide release is sticking out 1/4" ( pushed from right to left somehow).
Any ideas on how or why the slide release would come loose?

Here is my SWAG

upon doing the reload from slidelock, your trigger finger put pressure on the protruding end of the slidelock. You hit the slidelock, and it wasn't out of position too far yet, was able to load round....bang....click, ect....

that's my guess
