If the ammo, and the mags work in another XD, then there is something wrong with your gun, hate to say it but you might need to send it off,

Make sure all rounds are seated in the mag in the back CLOSEST TO THE REAR OF THE GRIP. I had this problem with my 45xd and I polished the ramp & put a .045 radius on the edge of the ramp. THAT STOPPED IT FOR GOOD,. I've fired 1500 rnds since without a jam.

do you check all you reloads w/ a case gage? and make sure your sizing die is set all the way down.
i had a similar problem, and it was my sizing die not my gun.

Check the OAL of the reloads. This kind of problem is usually caused by rounds that are too long. Check the reload manual for the OAL for the bullet weight and type you using.
