I've gotten hit with fragments of copper plating I'd assume, that or lead, from my friend's revolver when the dumbass would shoot it right next to me.

If stuff keeps coming back from your steel poppers, check the angle at which they are mounted. Mine never spit back.

Yes, we try to ensure the angles, distance, tarps (when used), other targets more forward, etc. For the most part, they work well to minimize fragments from flying around. The range is an indoor range and most times our setup is effective. But it seems like if there is 1 fragment flying back, it will hit me. Our guys have a great safety record though. No one has every been injured in any way. A little ping in the head with a small fragment doesn't bother me. It's amusing anymore. But in all seriousness, it reaffirms that you should always wear eye protection when you're in a range. We've had folks that come just to watch and our folks are insistent that they have eye & ear protection.

Got whacked pretty good by a hunk of x54R out of my M44 a while back. We were shooting a clutch plate at probably 75 yards or so.........yeah, I fire hear the DINGGGG and then hear something that sounds like a big zipper have time to say "What the fu...." SMACK right into my left arm. Had a pretty good sized bruise for about a week and half.

Some of these stories remind me of my first (or second?) trip to an indoor range when I started shooting a couple months ago.
I was in a stall with the closest other shooter being at least a few stalls away. I was loading a magazine when I felt something hit my right cheek, hard. I didn't yell out, but said out loud, "What the...?!" I looked down at the mag in my hands and thought, "Did a cartridge pop out and upward out of there and hit me?" I looked on the floor but saw no cartridge. "Must have been shrapnel," I thought, and finished loading. Shot for awhile longer and called it quits. Cleared my gun, secured it, put my shooting gear away and went to wash my hands. Looked in the mirror in the restroom and there's a good sized fresh scab on my cheek where I got hit. I hadn't even realized I was bleeding after I got hit! Penetrated the skin pretty well and took several days to heal. No wonder it stung. I've been hit with small fragments a couple times since then, but nothing like the first time.
