Talking Third time out, some questions

My third trip to the range was great in that I had two failures that forced me to work on my skills. The first was discussed here before, the slide locked back on my XD45 halfway through a magazine. My brother quickly killed any conspiracy theories as he said he saw my thumb hit the lock clearly. Apparently during the prior round I was adjusting my grip often, preventing the problem.
The second was the better learning experience. My slide failed to lock on the last round. I began to lower the gun, and then remembered about hang-fires and counted to 30 before clearing the gun. Not sure what caused the slide not to lock, but I was glad it happened because it helped me to burn safety procedures into my brain. Any ideas on the cause would be great.

thumb pushing up lock slide back early

thumb resting on slide stop prevents slide lock

I figured I must have hit the lock somehow. I am playing around with what grip suits me best. Overall I love the XD, it is a hell of a lot of fun to shoot
