XD40 Tactical Next to Last Round Failure to Feed
Ok, I've tried to search this issue (I'm fairly new to this board) but I'm having difficulty determining if a consensus has been reached about the failure to feed the next to the last round.
It appears the next to the last round begins to chamber correctly but because of position of the last round on the follower, the rim of the round being chambered catches and pulls the last round forward. As all other rounds do feed, I don't think it is a feed ramp / chamber issue. I would suspect it relates to the shape of the feed lips and the follower position of the last round. As I can't change the follower design, I'm thinking I should adjust feed lips to release the round earlier. Of course, I'm no gun smith, in fact.. I shouldn't even be permitted to own a dremel tool. See my stupid drawing... It happens on all mags, with both Remington UMC and Winchester White Box. I have 400 to 500 rounds through the gun. Any suggestions?

I had the exact same problem very first time I took my pistol out. Is your gun brand new? I credited my problem to being one of a few things. Limp wristing, stiff mag springs, pistol wasn't broke in yet, or I somehow hit the slide lock release and slowed the slide down when it cycled. Whatever the problem was, it never happened again after the very first time at the range. Springfield told me it takes at least 250 rounds to break it in, so it could have been from that. But if I still kept noticing it happen to send the mags in and they will replace them. Thankfully it went away. So, check that you aren't limp wristing, or hitting your thumb on the slide release when you are shooting. Other than that just break the gun in if you haven't shot much through it, and see how it goes from there. Keep your mags loaded for a few days to let those springs loosen up as well, might be the problem.

yes, I do but are the guide rod and spring the same on the service and tactical models? Once my gun loosened up, and I made sure I was holding it properly I had no more problems at all. Had about 5 the first day I shot it and it really made me mad. How many rounds do you have through it? Have you been keeping your mags sitting full for a few days? I really think it was combination of things that caused the problem, most due to the gun being new and me new to shooting that gun.

More of the Same...
I went to the range this morning.. same problem. I'm sure I'm not limp wristing the gun. It happened on more than one magazine. Internally the gun is stock.
3rd parties who are not insane witnessed the problem. I guess I will polish the ramp/chamber tonight..

I had the same issues for about the first 6 to 7 hundred rounds. I bought new magazines, marked them all and every single one I had FTF on the next to last round of each magazine. My suggestion and this was the suggestions given to me.
SHOOT THE **** OUT OF IT, polish the feed ramp, try a new magazine. Once mine broke in I had no issues. Just work on grip, make sure your rounds are seated to the rear of the magazine and rock and roll. I also did the DGR thing and that seemed to help a little to but I dont use the DGR now and I still dont have any problems.

Not sure but check the breach face, extractor, and firing pin hole for build up or other things that might be getting in the way of the round. My XD had a very rought breach and I was having the same problems with reloads I polished the breach face and the problem went away.
Limpwristhing causes failure to extract and eject not failure to feed.

Thanks to all you have responded.
I just polished the ramp and block a bit. I was wondering about polishing the breach face but I wasn't too sure that too I had that I could do it with.. but now that I think about it.. I have a 1 1/4 muslin wheel that I can put on my dremel and load it up with some fine compound. I'm off to the range in a little bit, I'll give a follow up report.
