Hah nice try. Of all my guns this one that I'll never get rid of. First CCW gun, lots of character (holster wear) and 5500ish rounds through it. It's like an old reliable truck you don't get rid of.

Forehead brass is typically a too strong spring or limp wristing or too light loads. These things can all interact and simulate the others, but isolate one item and you will be closer to the reason.

i've been hit with probably around 3 in the head/face. from what don has said limp wristing was likely my situation(s) problem. i've had, with a different gun, ended up with a circle from the powder as a third eye. lol, saw it later at night.

That has happened to me as well from time to time, mostly with WWB. I just continue on.

I almost got beat down at the indoor range the other day. My H&K P7 Squeezecocker uses the gas system to eject the shell casings(it uses a physical extractor as a backup and chamber loaded indicator).

Anyhow, it ejects shells at extremely high velocity at a distance of almost 4 lanes. Talk about some pi$$ed off shooters. Every one was griping about it. Fortunately, most were my friend.
