Anyone ever actually had a squib round in the barrel, not known it and fired again with a XD or XDm?

This was done on purpose during the testing of the HS2000 which the XD actually is ( Springfield changed the name to XD for importation ), no damage was apparently encountered.
Squibs in pistols don't scare me much but when I'm rockin and rollin my SURs it does give me concern.

Fortunately, no.
When practicing rapid fire, I always use factory ammo - either new ammo or reloads from places like Georgia Arms or Atlanta Arms. I trust them more than I trust me. Back when my powder metering system wasn't reliable, I did have several squibs in .40 S&W. As usual, I was practicing slow fire and caught the problem before I pulled the trigger again. A quick trip out to the car with a hammer and punch took care of the problem with no damage to the barrel. The bullet entered the barrel about 1.5" and stopped. I don't remember if the squib racked another round into the chamber. I kinda doubt it.
My bad for not visually checking EVERY case as it went around in the reloader.

My son is starting to reload and I hope we will catch any problems.. but it could happen.

Yes, it could. That's why a visual check with each handle stroke is important. Even with a big, hairy press like the Dillons (with all sorts of safeties), you still need to visually verify what's happening. A reliable power delivery system is a MUST! Also, something an experienced reloader told me: Nothing on your press should take more than 3 pounds of pull on the handle. If it takes more or feels strange - STOP, and evaluate the problem.

I have had one squib round in my Xdm using reloads. The bullet was stuck in the barrel pretty well. Used a wooden dowel to punch it out. it was very apparent that something went wrong after pulling the trigger. The gun also stayed out of battery.
When I was 18 I was firing cheapo slugs through my fathers 870 when I had a slug get lodged in the barrel. I failed to realize there was a malfunction... Racked the next shell into the chamber and proceeded to fire. There was a very large eruption of fire from them breech which burned my chest hair / eyebrows and some arm hair off. I dropped the gun immediately after assuming I had just lost some fingers or something.
The 870 fired the second round but it didn't even fully leave the plastic shell carrier. Some springs fell out of the gun and it was inoperable until we were able to replace the trigger and bolt assembly.

Yup. While using my 550 a few weeks back, I was down to the last empty brass to be loaded. My wife came into the garage to ask me something and I was moving that last brass to the bullet seating station. I looked down into the brass, as is my habit, and noticed that I had absent-mindedly double-charged the powder while talking to her.
Distractions while reloading are to be avoided. Alcohol is completely verboten.

I'd venture to say distractions are THE MOST dangerous things in a reloading shop. My shop is in a walk-in closet. The only media in there is a CD player. NO video!!! When someone knocks on the closed door, I finish whatever operation I'm performing, THEN see who it is and what they want. I have a three position Lee turret, and a Lee Pro 1000 presses. If anything goes wrong in a full 3-step reloading cycle, I clear all three positions at that point, and start over after the problem is corrected. That alone has prevented several double charges. If the powder delivery canister raises even a little (powder-thru flaring die) (like when I have to stop and reverse the advance a little), I remove that round. Understand I do NOT like to reverse the press, even a little (like when the empty shell falls over or something).
