I often carry two guns, so I often have two in the chamber.

Why? Because I can't plan how I might be attacked, so I have to be ready.

Life and death situations don't allow you time to go racking a slide. If your life is at stake you need to be ready NOW not in two seconds. Those time spent racking the slide is a perfect opportunity for an assailant to take you down easy.

I carry with one in the pipe. It does not matter if I am using my XD45, the 1911, or my 357mag. The vast majority of ADs and NDs are from people PULLING THE TRIGGER. The XD will not fire any other way. When I first carried it was a little unnerving to carry a loaded firearm, but after a couple of stops around town, the uneasiness went away.

Here is a simple example:
You are walking down the street with your family and a large dog jumps out at you from a yard, hedge, behind fence and knocks you to the ground. You raise your off hand to fend off the attack and the dog starts bitting it as you reach for your gun. With and empty chamber on a semi auto, how do you make it ready with a 50 to 75 pound dog standing over you trying to bite your neck face? This is not a personal experience but the first day I had my permit I witnessed a pit bull charge a neighbor. The dog stopped a few feet from him and turned and ran away but it happened so fast that if the dog had attacked, my friend would have been knocked to the ground before I could draw my XD. Very sobering.

It's tough to rack the slide after being shot.

Yes, I always carry my weapon ready to fire when needed!

Always,,,but I carry a 1911. I'd be a little leery of my XD that way. I have seen crazy things happen. ADs and NDs usually happen due to poor finger discipline, foreign objects, and general stupidity. Most I have read of occur during transitions into, some out of, the holster. These are usually due to poor finger discipline and foreign objects. It's pretty easy to understand why these transitions are the most dangerous. Therefore, I prefer a handgun with a manual safety. But, always one in the pipe.

seems to be a regular thing for ruger's now days...

on topic - always carry with one in the pipe.... racking a slide in a stress situation is one too many steps. XD always loaded, 1911 cocked locked and no thumb safety on
