Failure to Lock Back started happening to me regularly when I moved to a "thumbs-forward" competition grip on on my gun. I would swear that I was not touching the slide release, but the fact was that I was making an ever-so-slight amount of contact. Turns out that was enough.

I had to train myself to keep that thumb completely away from the release. Now I don't have the problem.

Personally, I don't think you should sweat this until it starts happening more than once-every-2000 rounds.

If it does start occurring regularly, remember that you can replace the slide release lever spring, as well as your magazine springs. You might also want to identify/number your mags so that you know if it is happening with a specific one.
