What does a little right and high mean?

Went to the range for only the second time yesterday with my XD45. I shot well and although shots were spread out around the COM area, I ended up chewing a hole through my target the size of a baseball, but the hole was just slightly high and slightly right of center.

What is the flaw in my form that causes high and right? I shot my CZ right after and did not have this problem, but I'm sure it's me and not the gun, as I do not remember a consistent high and right my first time out.
