It's possible for any gun to fire if an internal safety device is broken or compromised. It's why we keep our fingers off the trigger, ya never know.

It would be tough... The only two things I can think of that could cause this to happen, is if:
1 - The grip safety, and the striker safety somehow fell off/out of the weapon and you pulled the trigger.
or
2 - The striker itself snapped at the base, or in the middle causing the striker spring to send the end of the striker into the primer.
There is nothing that specifically "blocks" the end of the striker from hitting the primer. The safeties are:
1 - Striker Safety - This is a safety block that only lifts when you pull the trigger, and the trigger bar rotates the striker safety lever into the upward position. Lifting the striker safety out of the way so that the striker can freely move down the channel. In normal operation the striker should never touch the striker safety.
2 - Grip Safety - The grip safety locks the sear in the upward position until your hand is on the grip. The sear is unable to move downward to release the striker. So even if you drop the weapon, the grip safety will keep the sear locked, which holds the striker to the rear.
3 - Trigger Safety - The trigger safety locks the trigger into position against the frame of the hand gun. Unless your depress the trigger safety first, you cannot move the trigger, or trigger bar, to defeat the Striker Safety.

Yes.
The striker would have to break at the sear/striker safety engagement/block.
or
The sear and striker safety would have to fail.
Any mechanical device can fail, car breaks fail, accelerators fail, bicycles,etc...

My logic:
If one of my kids can get to my gun, I know they cannot rack the slide. The chances of my kids getting to my guns, in my opinion, is greater than the chance I'll ever be in a HD situation - I'll side with my kids safety.
I don't hide my guns from my kids or make guns a 'bad thing', every one of my kids from the 8 year old to my 3 year old know about guns, what they can do, and why I have them. They also know they are free to see them whenever they want, as long as I am around.
If my gun isn't on me, it's locked up and they do not know where the keys are.
Second point of logic, should a bad guy somehow get my gun only I will know it is not chambered with a round.
Go ahead and flame me all you want and say I have a short useless bat under the bed - that's fine, it's how I'm comfortable and I know my situation better than anyone.
