I pocket carry an LCP most days with an extra mag in the other pocket.
Don't have a mag carrier for the XDm, so I don't carry an extra when I have it.

I use a IWB crossbreed single mag pouch.
I carry an extra mag with me all the time not necessarily for the rounds but in case my main mag malfunctions or in a struggle bangs against the ground and the bottom plate cracks and my ammo goes spilling.
Ya never know!

WalMart carries in their firearm accessory section a universal magazine pouch. I just bought one for only $10.
It is a belt pouch. It sits high on the belt and is universal. It holds my single stack and double stack magazines. What I really like is that this magazine pouch looks like a knife or multi tool pouch. I have a couple of friends that just bought one of these too.

I try to. If I carry a bag I carry two. If I can I carry an extra but not always. But I always carry it inside my left cargo pocket in a magazine pouch along with a little flashlight. During the winter I carry an extra mag in my left jacket pocket and my phone in the right. Works out great for me. When wearing jeans it's harder but not impossible.

i carry 2 spare mags in the double mag holster that comes with the M&P 40s&w

Sometimes carry an extra mag but usually.
I usually have my XDm 9mm or .45 compact in my CBST & my Bodygaurd in a pocket holster. I'm probably going to start carrying my Sheild as soon as I order a CBST but will still carry a my backup as well.

I either throw mine in the small front jeans pocket and cover the top with my T shirt or I use this mag pouch. You can actually thread it horizontally onto your belt and it rides very nice.

Just about every failure can be fixed by dropping the mag, insert new mag, and racking the slide. While tap, rack, bang may work it may not.
