Religiously.

My reasoning is that I'm always conscious of that when I'm handling the weapon under "normal" circumstances. I know it's loaded and I handle it as such. In a life-or-death encounter, though, one never knows how exactly you will react. But it's not uncommon for folks to panic & forget that they need to chamber a round. Needing a weapon to defend myself, pulling it out and having it go "Click" is pretty much a worst-case scenario in my mind. I'd rather not have to worry about that and assume it's going to go "bang" (ammo issues, notwithstanding).

One in the pipe at all times. When I first started I didn't do this but then I took some training classes and got comfortable doing it.

We need a poll.

If it hasn't been said: if you need to present your firearm, then the situation means you need it to be ready for business.

I'm new to firearms as well and right now I carry without one in the chamber. I intend to try some of these practice techniques to get comfortable enough to carry one in the chamber without the thought in the back of my mind that something could go wrong.

Always be ready, train, get comfortable with your equipment.

I had a close call once where I almost had to draw my pistol and did not have one in the chamber. Going forward I always carry hot.

if you are not ready to carry hot you should wait until you are ready........

It's already been done. Like I said before this question comes up at least once a month sometimes with two active threads at the same time.
