I also noticed he appeared to tap the forward-assist a couple times....would an AR fire if just slightly out of battery (maybe a Glock-style case failure)? Any thoughts?

Squib would be correct.

This was posted on AR15.com awhile back only it was the whole vid instead of just the last few seconds.

They kept having ftfires and ftfeed. At one point they were discussing how so and so loaded the rounds so probably a combination of out of spec ammo and not breaking it down and verifying the bore was clear.

Dispite all of there stopages he just kept yanking the CH an shouting bad primer with out checking it then just raming another round in it and beating on the fa to force them in till finally it kbed.

Conclusion whoever made there reloads did a piss poor job, and an operator to busy trying to look cool in front of his buddy to check his weapon.

Case failure can do that also, so be careful about picking up range brass if you reload for your AR.
