sometimes the star washer digs into the rubber on those and it won't fit tight enough... i just throw a washer on mine...

As I stated earlier, the last two I installed were loose too. What I find more interesting is that both were also Spike's lowers and RRA parts kits. I wonder if it is the lower or the RRA grip screw? I'm betting grip screw. Anyway a 1/4" lock washer fixes the issue.
